It is important that people with a learning disability over the age of 14 are registered on their GP Learning Disability register and has an Annual Health check.
An Annual Health Check can help you stay well by talking to a Doctor or Nurse about your health and finding any problems early, so they can be sorted.
You do not have to be ill to have a health check – in fact, most people have their annual health check when they are feeling well.

NHS Dorset would like to hear about the patients and parents/carers experience of health checks to help us to make improvements and share positive experiences, as we want all checks to be good inclusive, supportive and accessible.
This survey Your Annual Health Check Survey (surveymonkey.co.uk) is for people over the age of 14 who have had an annual health check. An adult / carer / supporter may wish to share their feedback too.
